The following is a listing and usage description of current Contract Compliance program forms:

 

 

MSDGC FORM 104
Affidavit of Contractor or Subcontractor Prevailing Wage - Originals required and due with final payment request.
MSDGC FORM 105
Final Affidavit for Trucking Firms - This is an affidavit for trucking firms that are working on prevailing wage contracts but are exempt from payment of prevailing wage claiming the “de minimus” exemption.
MSDGC EQUAL EMPLOYMENT OPPORTUNITY (EEO) POSTER
The flier must be posted at all MSD construction project sites.
MSDGC FORM 147
EEO Form for Construction and Professional Services projects- This is a Company/Organization Data Sheet that is required of all Prime Contractors prior to award. It shall remain on file for one year.
MSDGC FORM 2004
Subcontractor Approval & Substitution Request (for all types of construction projects) - The form must be submitted in VCCS and approved by the MSD Prevailing Wage Coordinator before prior work begins on a project.

It is recommended this checklist be followed during the course of an MSD construction project to ensure compliance.
MSDGC STATE PREVAILING WAGE CHECKLIST
It is recommended this checklist be followed during the course of an MSD construction project to ensure compliance.
MSDGC SCHEDULE OF PAYMENTS & PAYROLL DATES
All contractors are required to submit this form to the MSD Prevailing Wage Coordinator before work begins on a MSD project. Contractors are responsible for their subcontractor's submittal of this form.
MSDGC PREVAILING WAGE NOTIFICATION TO EMPLOYEE
This form should be completed for each employee not covered by a collective bargaining agreement.
MSDGC PREVAILING WAGE COMPLAINT
This is the required form for prevailing wage complaints to be filed with MSD;s Contract Compliance Office.
MSDGC DAVIS-BACON POSTER
Notice of employee rights under the Davis-Bacon Act are required to be posted at all project sites where federal Davis-Bacon Act wage rates are applicable.
